Carer sensory profile with school companion questionnaires (SP-2)
Face to face 2 hour standardised sensory-motor assessment; Bruininks Oseretsky Test of Motor Proficiency (BOT-2), Visual-Motor-Integration Test (Beery VMI-2), test specific evaluation in Ayres Sensory Integration (EASI), clinical observations and activities of daily living (ADLs)
Comprehensive OT report, including test scores, analysis of assessment findings and recommendations
Feedback discussion following assessment

Individual OT package B:
Block of 6 individual 60 minute weekly OT sessions
Review meeting (telephone or Google Meet) with parents/school
TOTAL: £665
EHCP package C:
£95 per hour quantified within EHCP OT provision section F
(OT assessment package A would be an addition to this package if required)

Adoption Support Fund (ASF) package D:
£95 per hour quantified in initial quote available on request

Travel:
Travel miles are 65p per mile from postcode HP18.
Travel exceeding 30 minutes to the destination incur an additional 50% time fee based on the OT hourly rate.
