Occupational Therapy Referral Process
Please note that due to the volume of referrals, our waiting list is now closed until January 2026. We apologise for any inconvenience.
To manage our waiting list, we may close our referral process for a short period.
If you view 'Form over quota' when attempting a referral, please wait until after the 1st of the following month to complete your referral. Thank you for your patience.
New referrals are for 0-12 year old children only.
Please complete the referral form and we will be in touch to discuss your requirement and book an initial consultation.
Please read our policies, terms and conditions before signing the referral consent form, including our cancellation policy where fees for sessions remain chargeable if less than 24 hours notice is received.
Booking an OT assessment has approximately a 12 week waiting time.
Availability for individual OT sessions vary depending on the child's needs, although a block can usually be booked the following term after the initial consultation.
